This position will operate primarily as a Triage RN and will be responsible for triage calls from patients, assessing needs, giving appropriate clinical dispositions and facilitating referrals to primary providers, specialists, healthcare facilities and community resources. This nurse will also educate the client when appropriate regarding immediate care advice and preventative behaviors. Our organization maintains a HPSA rating of 17 and qualifies for the Nurse Corp Loan repayment program.
• Provides telephone and in-person nursing triage to patients to ensure they receive timely and appropriate care
• Refers all inquiries to appropriate resources or team-members. Consults with providers as needed
• Care plan development and management in collaboration with provider
• Documents all interactions, including nursing assessment and plan of care
• Patient health education
Skills & Knowledge
Superior nursing process skills. Ability to train and teach staff. Familiarity/experience with client interaction on the telephone. Involvement with quality improvement processes. Basic computer skills and comfort with Microsoft Windows operating system. Basic telehealth counseling skills. Knowledge of available basic community services. Basic typing skills (such as 30 WPM). Basic personal computer skills and comfort with Microsoft Windows® operating system. EHR experience, EPIC experience a plus. EHR experience, EPIC experience a plus. Knowledge of health insurance plans, standard office policies and procedures as well as regulatory requirements including CLIA and OSHA standards.
