SKILLS, KNOWLEDGE, EDUCATION, AND EXPERIENCE:
Associate's Degree in Nursing required; BSN preferred.
Must be licensed in the State of Oregon as a Registered Nurse.
2 (two) years of patient care experience preferred.
American Heart Association Basic Life Support for HealthCare Provider (BLS/HCP) certification required.
American Heart Association Advanced Cardiac Life Support for HealthCare Provider (ACLS) certification required or obtained within six months of hire – not required for Pre-Surgical Screening.
Additional certifications as required by unit:
ED, PEDS, ICU, OR: American Heart Association Pediatric Advanced Life Support for HealthCare Provider (PALS) certification required or obtained within 6 months of hire.
ED: Trauma Nurse Core Curriculum (TNCC) desired with the goal of obtaining within 1 year of hire date.
OB: Neonatal Resuscitation Program (NRP) required or obtained within 6 months of hire date.
Uses the Nursing Process in Assessment, Planning, Implementation and Evaluation of patient care demonstrated by:
Completes admission assessment, care planning and documentation within expected time requirements
Uses critical thinking skills to recognize/respond to urgent or changing patient needs and initiates action to correct, reduce, or prevent risk. Bases decisions and actions on scientific principles/logic, effective/successful experience, research findings, and/or established standards and protocols
Plan of care is developed with patient/family input, individualized, age appropriate, has clearly defined goals and is achievable.
Facilitates discharge-planning process and assures discharge plan is reviewed and education is provided to patient/family prior to discharge.
Prioritizes and implements care of patient according to patient needs and care plan goals
Individualizes, implements, and documents patient’s education to meet expected patient outcomes.
Documentation is consistent, clearly identifies progress toward goals, indicates the efficacy of medications and therapies, notification of provider(s) with changes in patient condition changes, etc.
Effectively delegates duties to care givers (LPNs/CNAs), and manages shift work duties in an organized and consistent manner demonstrated by:
LPNs/CNAs perform care delivery within their defined scope of practice
Patient care duties are communicated clearly to care givers and monitored throughout assigned shift.
Mentoring and teaching care givers to enhance skills, understanding of care modalities and communication expectations for best patient outcomes
Communicates to PCM areas of concern regarding concerns with care giver delivery of patient care, education/training needs and communication effectiveness
Medication Administration Effectiveness demonstrated by:
Following established safe medication administration policies.
Consistently uses the “Read Back Verbal Order” documentation for all verbal/telephonic orders received
Completes information gathering, reconciliation process, physician notification and discharge instructions required in the medication reconciliation process
Provides verbal and written information to patients regarding new medication’s actions and side effects, and assures patient is educated on discharge medications.
Completes narcotic counts and periodic narcotic audits per policy.
Resolves 100% of Pixis end of shift discrepancies prior to leaving the nursing unit
Actively involved in Quality Improvement and best practices initiatives demonstrated by:
Participation in organizational/system Evidenced Based Practice Initiatives and unit specific quality activities
Participation in organizational quality committees or work groups including best practice, policy/procedure and order set development
Working knowledge of CMS Core Measure quality initiatives, documentation requirements and assesses completion of quality indicators for those patients assigned (such as CHF discharge instructions, Vaccinations, etc.)
Performing chart review and/or other data, collection for internal and external reporting processes as assigned.
Effective Interpersonal and Communication skills demonstrated by:
Consistently uses the SBAR format for all hand off communication between shifts, transfers in level of care (i.e. Emergency to Medical Unit), etc.
Accurately completes shift report per unit guidelines including documentation of staff work assignments, etc.
Communications with physicians and other care staff is timely, consistent, pertinent, and appropriate to care needs of patient.
Hand written communication is legible and understandable
Critical Values reports are relayed and documented to physician in a timely fashion
Interdepartmental collaboration to maximize patient services effectiveness in contributing to overall organization effectiveness and success demonstrated by:
Achieving patient outcome goals through collaboration with CM/UR/Clinical documentation specialists
Facilitating interdepartmental transfers, patient flow, and initiation of therapies in a timely fashion
Optimizes unit workers and supply resource utilization through continued communication with PCM and Supervisors regarding daily unit needs, shortages, or excesses of resources.
Assist PCM to achieve unit productivity targets and efficiency in supply and workers by identifying and recommending ways to improve work processes.
Maximize nursing documentation and communication to physicians to clearly identify conditions present on admission to proactively limit adverse financial risk to hospital
Facilitate timely and effective discharge or transfer of patients to the next level of care
Professional Development demonstrated by:
Effectively represents appropriate practice standards and the perspective of professional nursing in unit and organizational meetings
Maintains professional working relationships with physicians, ancillary medical staff, nursing and ancillary clinical staff
Active involvement in professional training activities as demonstrated by serving as mentor , preceptor, and role model to nursing students and new and existing Saint Alphonsus nursing employees
Maintains/develops professional skills/competencies within the Emergency Department and serves, as clinical expert to other RNs to assure that standards of nursing practice and patient care are met or exceeded.
Promotes collaboration with physicians in the provision of patient care
Identifies and works towards own professional growth and developmental goals
Attending at least 70% of unit staff meetings, and reads and signs 100% of posted meeting minutes.
