The incumbent serves as a clinic patient advocate by answering patient inquiries and resolving patient concerns at a local level. The Orthopedics Nurse Practitioner (NP) or Physician Assistant (PA), practices in the Orthopedic Clinic, in the Operative Care Division in Portland, Oregon. The Orthopedic NP is an independent member of the Medical Staff, with delineated clinical privileges. The NP/PA in Orthopedics is a member of an interdisciplinary team providing medical services to patients in need of Orthopedic care. The responsibilities may include, but are not limited to the following: Performs simple laceration suturing; venipuncture; arterio-puncture; urinary catherization; suture removal; administration of IV fluids; administration of intramuscular, subcutaneous and intradermal injections; administers local nerve blocks; arthrocentesis, and joint injections; cleansing and debridement of wounds as needed. Works with or in the absence of the Orthopedic Cast Technician, to include: application and removal of plaster and fiberglass casts; treatment by alignment and immobilization of non-displaced and minimally displaced fractures of the extremities; and fitting orthopedic appliances, i.e. braces, splints and assistive devices. Responsible for initial screening of patients in outpatient clinics; screenings may include history, physical exams, ordering and reviewing laboratory tests and x-rays, electrocardiograms, and other routine diagnostic tests. Additional responsibilities include: takes an active role in interdisciplinary care coordination, communicates relevant patient data. Formulates, initiates and facilitates communication with personnel responsible for patient's hospital and clinic services and acts as a consultant to nursing and medical staff. The NP/PA will be available for assisting in the operating room as possible.

Education or Training: The candidate must meet one of the following: (1) A bachelor's degree from a physician assistant (PA) training program which is certified by the Accreditation Review Commission on Education of the Physician Assistant (ARC-PA); or (2) Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a bachelor's degree in a health care occupation or a health-related science; or (3) Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a period of progressively responsible health care experience such as an independent duty medical corpsman, licensed practical nurse, registered nurse, medical technologist, or medical technician. The duration of approved academic training and health care experience must total at least 5 years.
Certification: As of March 12, 1993, certification by the National Commission on Certification of Physician's Assistants (NCCPA) is a requirement for employment. This requirement does not apply to non-certified PAs on VA rolls as of that date.
Physical Standards: Drug Testing and Physical: All applicants tentatively selected for VA employment is a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA.A pre-employment physical examination shall be made to determine the physical and mental fitness of candidates for appointment in the VA.Physical examination must be satisfactorily completed.
English Language Proficiency: As appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Note: This occupation may not be used to employ physicians or other health care providers who do not meet the applicable qualifications standards for their occupation. All physician assistants must meet the basic requirements listed above.

Full unrestricted Licensure, Certification, or Registration required
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 years of nursing practice/experience.
Nurse Practitioners. On and after March 17, 2009, registered nurses appointed or otherwise moving into these assignments must meet and maintain the following additional qualifications. This includes employees appointed before March 17, 2009, who obtain such qualifications on or after March 17, 2009.
Nurse Practitioners. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.
Preferred Experience: At least one year of clinical experience, characterized by strong history and physical assessment assignments.

Physical Requirements:This position requires visual activity, keen hearing, clear distinctive speech, and manual dexterity. This position requires a combination of walking, standing, stooping, kneeling, bending, pulling, and pushing with potentially long periods of sitting. Transferring patients and objects may occasionally be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or during patient procedures and care. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Use of fingers, both hands, and near vision is required.
