Opportunity The Pediatric RN Care Coordinator provides support to patients and care plan management utilizing the nursing process of assessment, planning, intervention, and evaluation within the Patient Centered Primary Care Home model. The goal is to provide individualized education, coaching and follow-up to improve patients' self-management skills. Interventions are intended to help patients and patient populations adhere to treatments and maintain their quality of life with chronic disease and/or complex illness. They manage a panel of complex patients and lead team huddles to present these patients. The RN Care Coordinator will lead and facilitate the coordination of care between others involved in the care of the patient, including the patient's primary care team, medical specialists, hospitals and health plans. Responsibilities ' Coordinate the care of complex patients using evidence-based practice ' Follow up and coordination of plans of care, including referral support and prior authorization coordination ' Utilize standing orders to manage the care of patients (including, but not limited to anti-coagulation) ' Patient health education Skills & Knowledge Superior nursing process skills. Ability to train and teach staff. Familiarity/experience with client interaction on the telephone. Involvement with quality improvement processes. Basic computer skills and comfort with Microsoft Windows operating system. Basic telehealth counseling skills. Knowledge of available basic community services. Basic typing skills (such as 30 WPM).
