POSITION SUMMARY: The night/on-call nurse is responsible for responding to all of Coastal Home Health and Hospice (CHHH) patients and their family members, as well as making referrals and community inquiries when the office is closed. The On-call nurse provides: Clinical and psychosocial interventions for patients and their families. Skilled nursing care. Collaboration with providers within and outside of CHHH to maximize patient outcomes. The on-call nurse consults with the case manager, clinical director and physicians as necessary ESSENTIAL DUTIES AND RESPONSIBILITIES: May include, but are not limited to the following: 1. Is constantly available by cell or telephone during the hours of 5:00 pm to 8:30 am, Monday through Friday. 2. Responds to answering service within 5 minutes and be ready to travel in 10-15 minutes. 3. Conducts admissions as necessary. 4. Conducts physical and psychosocial assessments of patient's and family's needs after a. hours and determines intervention. 5. Provides interim guidance to the patient and his or her family regarding diagnosis medications, treatments and patient progress. 6. Documents care in a thorough and timely manner. 7. Works closely with IDG and home health team to provide continuity of care and support for the patient and family. 8. Triages patient phone calls and provides skilled nursing care as needed. 9. Collaborates with vendors to obtain DME, supplies and medications. 10. If needed, make arrangements for patient transfers to the hospital by calling report to hospital. 11. Collaborates with hospital, SNF, AFH or any assisted living facility to adjust interventions and plan of care as well as provide teaching/training to unlicensed personnel or family member. 12. Attends mandatory staff meetings as required. Ability To- Interpret orders and carry out instructions while exercising judgment within the scope of the position and established nursing practices. Work with limited supervision in an independent setting, while able to recognize situations in which supervision and guidance is required. Work with computers/programs and able to learn basic software applications. Communicate therapeutically with the patient, family, and public and maintain a compassionate, empathetic attitude. Education, Certification, and Licenses ' Graduate of an approved school of professional nursing. Must hold a current unencumbered RN nursing license to practice in Oregon.
