WorkSource Oregon QMHA ACT Case Manager in Clatsop County, Oregon
The primary purpose of this position is to work with the most acute and highest level of needs of individuals with severe and persistent mental illness. Assertive Community Treatment (ACT) provides support to individuals who are diagnosed with a severe mental illness and whose needs have not been well met by more traditional mental health services. They are identified by high utilization of hospitals and correctional facilities. The employee in this position will work and collaborate with the ACT multi-disciplinary team to deliver integrated services that meet recovery goals and align with the OCEACT Fidelity Scale. The ACT team delivers services to individuals in community settings; allowing individuals to be met where the individuals live, work, and socialize. Specific to Open Door case management are service planning and coordination, family and social relationships support, referral to substance abuse treatment and support, housing assistance, referral to supported employment and education, wellness self-management, problem solving, skill development, (including basic personal care and safety, grocery shopping and cooking, purchasing and caring for clothing, household chores, using transportation, and other community resources), health education and support, medications access, financial management, and empowerment and self-help. Requirements: 'Bachelor's degree in Social Work, Behavioral Health, Psychology (Trauma informed care) or Counseling or an acceptable combination of education and experience to be certified as a QMHA. '2 years' prior experience working with people diagnosed with serious mental illness. 'Valid Oregon or Washington Drivers' license 'Excellent oral and written communication skills 'Strong attention to detail, highly organized and ability to multitask. 'Knowledge of Health Insurance Portability and Accountability Act (HIPAA) and importance of maintaining confidentiality. 'Ability to learn and adopt the policies and procedures of the agency. 'Ability to pass criminal background check Preferred: 'Knowledge of the DSM-5 'Knowledge of civil commitment process 'Experience with electronic health records 'Knowledge of Trauma Informed Care 'Skilled at remaining calm in stressful situations and de-escalation techniques.
