The qualified candidate must have experience in Home Health, Start of Care/Oasis and licensed Registered Nurse in the State of Oregon. Prefer experience with HomeCare HomeBase EMR.
This position is responsible for implementing the Company’s continuous Quality Assurance and Performance Improvement (QAPI) Home Health/Hospice plan and program. The QAPI Coordinator provides support to the QAPI Manager. The role works collaboratively with all members of the Quality team and will support initiatives that aim to strengthen the organization’s QAPI activities and regulatory compliance. The QAPI Coordinator will have responsibility for auditing, reviewing and assessing/ trending data related to regulatory compliance and QAPI, as well as helping with implementation of corrective action plans and development of policies and procedures to address deficiencies within these programs. The role assists the QAPI Manager with compliance-related education efforts and with any quality improvement projects within the Quality programs.
Fulfills the QAPI program’s audit requirements
Participates in the organization-wide Quality Committee and review of all monthly, quarterly, and annual reports related to QAPI Program efforts
Participates in the analysis of statistical reports based on the results of utilization reviews, clinical record audits, post-payment compliance audits, customer satisfaction surveys, and the continuous quality management plan
Assists in the root cause analysis, development and implementation of corrective action plans for trends and deficiencies identified through utilization review, clinical record audit, third-party payer-claim denials, client satisfaction surveys, and the continuous quality management plan
Assists in the development of plans of action including process of care investigation, performance improvement plans, best practice implementation and monitoring, and project evaluation
Effectively assists clinical providers in problem solving and serves as a resource to achieve positive outcomes
Participates in accreditation activities as well state and federal survey preparedness and follow up
Participates in the development of clinical data to be utilized for standardized outcome management
Identifies patients with highest risk factors to proactively improve outcomes
Ensures the accuracy of OASIS and outcomes by educating staff, performing documentation reviews and training and monitoring case weights
Assists in the execution of new and updated best practices with local agencies and provides education to field staff as needed
Keeps all data collected centralized and organized manner for prompt retrieval
Develops agenda for Quality Committee and facilitates meeting
Assumes accountability and responsibility for accuracy or quality assessment data
Participates in orientation to the program, regulatory compliance, and the clinical documentation system (in conjunction with super users)
Participates in required clinical and quality
Supports mission, values and goals of Signature
Current Registered Nurse licensure from an accredited nursing school
Bachelor’s degree preferred
Prefer experience (2+ years) in a medical, surgery or home health setting
Prefer experience with quality and/or process improvement
Self-directed, able to work independently and demonstrates high level of motivation
Superior organizational skills and ability to prioritize appropriately
Strong interpersonal skills with the ability to communicate effectively, both in written and verbal form
Ability to analyze and present data in a clear, understandable way
Strong computer skills (Word, Excel, PowerPoint), with ability to utilize spreadsheets and manipulate data
Demonstrated ability to complete work through collaborative efforts with a broad group of individuals
Ability to complete work and meet deadlines
