HUD-VASH is an intensive case management program for chronically homeless veterans aimed at providing psychosocial services to veterans while residing in stable housing through HUD's Section 8 voucher system. There are two positions within the Roseburg Health Care System, one in Roseburg and the one in Eugene.

Responsibilities

The Social Worker in this position functions as the Case Manager for VA Roseburg Healthcare System Housing and Urban Development / VA Supportive Housing (HUD-VASH). The HUD-VASH Program falls within the Healthcare for Homeless Veterans (HCHV) Programs of Social Work Services.

Some of the duties of the position are:

  • Providing case management for a caseload of veterans in the HUD-VASH program.
  • Reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.
  • Site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes.
  • Verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation
  • Ensuring that homeless veterans are considered as primary participants for program acceptance.
  • Collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.
  • Providing HUD-VASH case management.
  • Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies.
  • Complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the HUD-VASH Case Manager.
  • Participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH.
  • Following local policy and SOP for the HUD-VASH Program. This includes following policies and documentation requirements as required for CARF and JC accreditation.
  • Program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
  • The incumbent takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.
  • Document all contacts with patients/families in the medical charts
  • Completes statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies. Work Schedule: Monday- Friday 8:00am to 4:30pm Fi*nancial Disclosure Report:* Not required

Qualifications

Basic Requirements: * United States Citizenship:Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. * Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduate of school of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. License:This position is at full performance, so you must have current licensure or certification in a state at the independent practitioner level. Preferred Experience: Independent Clinical Experience, work independently, experience with homeless clientele, one year post licensure experience

Grade Determinations: * GS-11 Social Worker: Experience, Education and Licensure. The GS-11 full performance level requires completion of a minimum of 1 year of post MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR, In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs in subparagraph (b) below.

Demonstrated Knowledge, Skills, and Abilities

  1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
  2. Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
  3. Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture.
  4. Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
  5. Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. This requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
  6. Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families. Social workers must practice within the bounds of their license or certification. For example, some states may require social workers providing psychotherapy to have a clinical level of licensure.
  7. Ability to provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate students. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.
  8. Ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.
  9. Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking, especially those programs in use by VHA.

Physical Requirements: The work is primarily sedentary. Typically, the employee will sit to do the work. However, there may be some walking, driving, standing, bending, carrying light items (such as books, papers), accessing transportation, and/or driving a government vehicle. The work requires patience and control of emotions. The work may require occasional use of appropriate techniques to physically restrain clients who present a danger to self or others.
