Providence Speech Therapist in Hillsboro, Oregon

Providence is calling a Speech Therapist (1.0 FTE, Days) to Elderplace Beaverton.

We are seeking a Speech Therapist - Elderplace Beaverton Medical Services that will manage and provide skilled speech/language pathology to geriatric participants/clients with a wide variety of diagnoses and disabilities, taking into account individual participant's special physical or age-related needs. Requires skills in participant/client evaluation and treatment, as well as professional consultations to achieve maximum participant/client independence in accordance with individual's functional level and potential.

In this position you will have the following responsibilities:

  • Provide appropriate, timely and effective evaluation and re-evaluation of participants:

  • Evaluate participant/client's current cognitive, communicative and swallowing status and develops individualized/measurable treatment goals and establishes interventions as appropriate to participant’s age, condition, diagnosis, response to treatment and learning needs

  • Incorporate participant and family as appropriate in goal setting

  • Re-evaluate and update goals in a timely manner

  • Plan and implement a comprehensive and individualized speech/language pathology treatment plan:

  • Select safe and effective treatment procedures as appropriate for the participant/client's age, condition and diagnosis

  • Modify and progress the program in response to changing participant/client needs

  • Instruct participant/client and caregivers to assist in achieving treatment goals

  • Terminate treatment as appropriate in relation to participant/client’s age, condition, diagnosis and functional performance / outcome expectations and discharge criteria

  • Communicate frequently and effectively in the professional environment with participant's, caregivers and others:

  • Provide and maintains accurate, thorough, legible and timely participant related documentation, handwritten and/or computerized, in accordance with department policy and procedure

  • Communicate frequently and effectively with others involved in participant/client’s care regarding scheduling, treatment plans, progress and discharge planning

  • Provide non-participant related documentation accurately and in a timely manner to Day Center Supervisor / Operations Manager / Rehabilitation Program Coordinator

  • Demonstrate professional behavior and pursues professional development:

  • Maintain and enhances personal expertise through appropriate professional educational experiences

  • Maintain required program competency in environment of care, age-related care, CPR and other required system trainings

  • Maintain a high standard of professional accountability with respect to appearance, demeanor, safety, ethical and legal issues

  • Promote injury prevention: in-servicing employees, families/caregivers and community groups as appropriate, and identifying and reporting potential concerns

  • Keep informed of health care issues, changing department procedures, and other department information by attending staff/team meetings and/or reading staff meeting minutes, e-mails and newsletters

  • Participate in department/service goal setting, QI activities, and Mission Practicum activities

Qualifications:

Required qualifications for this position includes:

  • Master's Degree -OR- Ph.D. from an accredited Speech/Language Pathology Program -OR- equivalent education/experience

  • Current Certificate of Clinical Competence from the American Speech-Language-Hearing Association or eligibility

  • Speech/Language Pathology licensure through the State of Oregon, or eligibility

  • American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification at time of hire

  • Occasionally a position will require specialized speech/language pathology experience to meet a particular department need

Preferred qualifications for this position include:

  • Coursework/Training: Specialized speech/language pathology training to meet a particular department need
