About the Program Our Outpatient Services Program provides initial assessment and evaluation, treatment plan development, and provides clinically indicated outpatient therapy services, which helps maintain continuity of care for members within our community. This program will have behavioral health treatment teams that are fully integrated with primary care to treat the whole person in an efficient coordinated manner. About the Position We are seeking a qualified mental health professional to fill the Part- Time Clinical Intake Specialist position at our Downtown Clinic in Medford, OR. This position provides support for the mental health intake process at the outpatient facility. We are looking for someone that can organize work, set priorities, meet critical deadlines, and follow up assignments with a minimum of direction. Work Schedule: Monday ' Friday, 10:00 am'2:30 pm Primary Responsibilities: This position will perform a variety of duties including; triage, conducting initial screening interviews to confirm eligibility, determine appropriate services needed, conduct mental health assessments when possible, provide referral information and assist with connecting and coordinating care, develop initial treatment plans, and coordinating case activities with clinical managers. Qualifications MUST be QMHP Certified through Mental Health & Addictions Certification Board of Oregon (MHACBO) -OR- MEET eligibility qualifications to register with MHACBO MUST meet State of Oregon QMHP credentials: Master's degree in Psychology, Counseling, Social Work or related field; Bachelor's degree in Nursing and be Licensed by the State of Oregon Bilingual in Spanish is a PLUS. Successful applicants MUST also have a valid Oregon driver's license, maintain a safe driving record, and the ability to pass a DHS criminal background check.
