Lifeways is currently seeking a Full-time Community Health Worker to provide services for our Umatilla County Team ESSENTIAL JOB DUTIES AND RESPONSIBILITIES: - Provide direct services within the scope of a Community Health Worker. - Assist individuals involved with DHS' Self Sufficiency Program to improve their health and increase the capacity of the community to meet the health care needs of its residents and achieve wellness. - Provide health education and information that is culturally appropriate to the individuals being served. - Referral tracking and management for individuals in multiple settings to access health & social services; including mental health and/or alcohol and other drug abuse treatment as appropriate. - Advocate for clients in navigating interactions/services with community agencies. - Provide client centered motivational barrier removal assistance. - Provide input to assist the Family Coach in designing TANF case plans that are coordinated with client treatment plans. - Coordinate participant engagement. - Data tracking, case assistance and regular reporting detailing attendance, activity/service received and completion rates - Participate in weekly client staffing and attend program meetings. - Offer peer counseling and guidance on health behaviors. - Provide timely and comprehensive follow up with providers to ensure clients are connected with services. - Increase access to primary care through culturally competent outreach and enrollment strategies. - Cultural mediation between communities and health and human service systems. - Maintain client records per all HIPPA/FERPA and confidentiality regulations. - Limited management of budget, reporting of expenditures to employer. - May assist with scheduling of medical appointments and completing health assessments. - May assist with Medicaid and Medicare applications and housing for clients.
